Seen a publicly documented AI governance failure that belongs in the Library? Contribute it here. This is the entry point for the Contributor role: submissions go into an editorial review queue — not straight to the catalogue — and every published case is attributed to its contributor.

Please submit only publicly documented incidents (news reports, court filings, regulator actions, academic studies). Include a source link. Avoid confidential or personal information about private individuals. See the full submission guidelines.

Names are used for editorial verification. The public catalogue masks identities by default.

Published cases are credited to the contributor. Your email and phone are used only for verification and follow-up — they are not published. See our privacy policy.

Read the contributor terms

By submitting a case you confirm that:

  • it is based on publicly documented, verifiable information, and is accurate to the best of your knowledge;
  • it contains no confidential, leaked, or private personal information, and nothing defamatory;
  • you grant WEKID™ LLC a non-exclusive, royalty-free license to review, edit, publish, and incorporate your submission and any case derived from it, with attribution to you where published; and
  • you retain authorship credit for your contribution.

This acknowledgment applies to individual case submissions. Sustained contributors are onboarded under the full Contribution Policy and Contributor License Agreement.
