Submit an AI failure case

Contribute a publicly documented AI governance failure to the WEKID evidence corpus. This path is open to everyone. Submissions enter a governed editorial review queue — never the live catalogue directly — where they are checked for evidence quality, relevance, attribution, and masking requirements before publication.

Please submit only publicly documented incidents (news reports, court filings, regulator actions, academic studies). Include a source link. Avoid confidential or personal information about private individuals. See the full submission guidelines.

Choose the closest match to keep the catalogue consistent. Use “Other” only if no listed sector fits — a reviewer will confirm or normalize it.

Names are used for editorial verification. The public catalogue masks identities by default.

Published cases are credited to the contributor. Your email and phone are used only for verification and follow-up — they are not published. See our privacy policy.

Read the contributor terms

By submitting a case you confirm that:

  • it is based on publicly documented, verifiable information, and is accurate to the best of your knowledge;
  • it contains no confidential, leaked, or private personal information, and nothing defamatory;
  • you grant WEKID™ LLC a non-exclusive, royalty-free license to review, edit, publish, and incorporate your submission and any case derived from it, with attribution to you where published; and
  • you retain authorship credit for your contribution.

This acknowledgment applies to individual case submissions. Sustained contributors are onboarded under the full Contribution Policy and Contributor License Agreement.
